2. Assessment
- Our experienced Care Managers will talk through your needs with you and help you to draw up the most suitable & appropriate care plan.
- We will email or post you on your individual Care Plan Costing, including information on our services and info on how to pay
- If you wish to proceed with the Care Plan our Client Services Manager will set up an assessment with you at a time that is convenient for you.
- At the assessment we will go through in detail the service you wish to receive from us (this can be anything from what is eaten for breakfast, to a more complex talk about the level of Personal Care that will be required).
- During the Assessment our Client Services Manager will make you aware of the different services you may also be able to access in your community (ie Community OT, Public Health Nurse Services, Day Centres etc)
- The Assessment is a vital stage in setting up the service successfully so it will require you allowing an hour or two for it to be completed effectively.
3. Care Plan
- Based on your needs & requirements highlighted in the Assessment we will then select a care assistant (or team of Care Assistants) to best suit your needs.
- Please note that if your service runs over 7 days with morning & evening visits you will require a team of up to 4 Care Assistants to ensure that you will always have a service and that there will be no gaps if ever a Care Assistant is unwell or on Annual Leave. This team will be introduced to you before commencement of service.
- Once the Care Assistants have been selected our Client Services Manager will set up an introductory visit with you.
- The Client Services Manager will supply you with your client folder. This contains your Client Care Plan, a Daily Diary (in which our Care Assistants must record the service they've provided to you after each visit), a copy of our policies and procedures and information on our 24hr 365 day a year On-Call Service
4. Care
- At Inhome Care we have a vast amount of experience in setting up services successfully. We have found time and again that if a service is set up well from day one it does tend to have a much better chance of running successfully for the client for longer.
- To this end our Client Services Manager will be on hand frequently for the 1st & if necessary 2nd month of the service to ensure that your service can be started as smoothly as possible. We understand that a lot of clients & their families may never have had to receive care or provide care and we will be there to support you with this new stage in your life.
- Once you're happy with how the service is going our client service manager will then carry out monthly check in visits and you can call the office at any time to discuss any adjustments or improvements to your service. There is an experienced Care Manager at the end of the phone 24hrs a day 365 days per year if you have any urgent queries.
- We will carry out official evaluations of the service twice a year.
- All individual Care Plans will be updated as needed based on feedback from Clients, their families & Care Assistants
- We welcome any & all input on the service positive or negative. Input from our clients is vital for us to continually improve the quality of all of the services we provide.
